{"id":3,"date":"2023-04-17T10:59:38","date_gmt":"2023-04-17T10:59:38","guid":{"rendered":"https:\/\/southernskinclinic.co.nz\/?page_id=3"},"modified":"2023-06-15T09:13:47","modified_gmt":"2023-06-15T09:13:47","slug":"privacy-policy","status":"publish","type":"page","link":"https:\/\/www.southernskinclinic.co.nz\/?page_id=3","title":{"rendered":"Privacy Policy"},"content":{"rendered":"\n<p class=\"wp-block-paragraph\"><strong>Privacy Statement \u2013 Southern Skin Clinic<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>(Health Information Privacy Code 2020 Policy)<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Objective<\/strong><strong>: <\/strong>To comply with Health Information Privacy Code 2020 when collecting, using, storing or disclosing information about patients\u2019 health or the treatment that they are receiving.<strong><\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Collecting health information<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">We collect personal information from you, including information about your:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>name<\/li>\n\n\n\n<li>contact information<\/li>\n\n\n\n<li>location<\/li>\n\n\n\n<li>interactions with us<\/li>\n\n\n\n<li>billing or purchase information<\/li>\n\n\n\n<li>relevant health records<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Using health information<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">We collect your personal information in order to:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>provide appropriate healthcare within our scope of practice<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Sharing health information<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Besides our staff, we share this information with:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>relevant local and regional healthcare organisations, including Southern Community Laboratories, Te Whatu Ora, WellSouth Primary Health Network, patients\u2019 registered general practitioners, and hospital specialists&nbsp;in order to&nbsp;provide appropriate healthcare services within our scope of practice.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">We will not disclose a patient\u2019s health information without his\/her consent (or the consent of his\/her representative) unless we reasonably believe that it is not possible for us to get the patient\u2019s consent and:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>the disclosure is for the purposes of the patient\u2019s treatment (e.g. a referral);<\/li>\n\n\n\n<li>the disclosure is to the patient\u2019s caregiver and the patient hasn\u2019t objected to the disclosure;<\/li>\n\n\n\n<li>it is necessary to disclose the information to prevent a serious and immediate threat to the patient or another person\u2019s life or health;<\/li>\n\n\n\n<li>the disclosure is made for the purposes of a criminal proceeding;<\/li>\n\n\n\n<li>the disclosure is to a social worker or the police and concerns suspected child abuse<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Providing some information is optional. If you choose not to provide&nbsp;name, date of birth, national health identifiers or relevant medical background, we will&nbsp;be unable to provide some healthcare services.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Storage of health information<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">We keep your information safe by&nbsp;storing it in a fully-encrypted patient management system&nbsp;and&nbsp;it is only accessible to staff directly involved in your care.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">You have the right to ask for a copy of any personal information we hold about you, and to ask for it to be corrected if you think it is wrong.
